YOUR FIRST SESSION

First of all, thank you so much for making an appointment with Tyler. As the title implies, this page is intended to explain everything you need to prepare for your first session.

To begin, you will need to download the necessary paperwork. In order to begin treatment, you will need to print and complete these six forms (the research informed consent form is optional):

Release of Information

You can bring the forms (at least the four mentioned above) with you to your first session or email them to tyler@tylerorr.com.

Tyler’s office is located at 110 Woodland Ave, Chattanooga, TN 37405. There is a church behind the building that allows us to park there if space is available. Usually spots are open. Please do not park in the hair salon’s parking lot. Tyler shares space with Tonal Spine Chiropractic. You will see their logo on the door. Once you enter the door, you will walk up a flight of stairs. There are some chairs at the top of stairs. You are welcome to wait there and Tyler will greet you as soon as he’s available.
